Bendigo Loddon Primary Care Partnership operates on the traditional lands of the Dja Dja Wurrung, Taungng 

and Barapa Barapa people and recognises them as the traditional owners. We pay our respects to leaders and elders past, present and emerging for they hold the memories, the traditions, the culture and the hopes of all 

Dja Dja Wurrung, Taungurung  and Barapa Barapa people.

Strategic Model - Our House

Bendigo Loddon Primary Care Partnership's (BLPCP) Strategic Model, also referred to as Our House, was developed in conjunction as the Strategic Plan 2013 - 2017 to clearly visualise the strategic operations and directions for BLPCP. 

 

The roof, overarching everything BLPCP does, includes the gender, health equity and social inclusion lenses, diversity, together with a Population Health Planning approach.

 

The foundation is made up of BLPCP's partnerships governance, continuous quality improvement and information technology.

 

Workforce development and service coordination are the pillars, supporting all areas of BLPCP's work.  

 

The Social Determinants of Health are the conditions in which people are born, grow, work, live, and age, and the wider set of forces and systems shaping the conditions of daily life. These form the bricks and mortar, and the base for all BLPCP work. 

 

Finally, the windows are BLPCP's core domains; Prevention and Access, Equity and Integration.
